Mission: 
To improve the value of health care for all people in Greater Columbus by catalyzing collaboration among public and private partners: Providers, behavioral health, hospital systems, social service agencies, public and private payers, employers, government, public health and patients/consumers.

At Healthcare Collaborative of Greater Columbus (HCGC) we strive to improve the value of healthcare for all people in Greater Columbus by catalyzing collaboration among public and private partners.

Our successes build from a collective impact model that starts with collaborative learning. By bringing broad, diverse community partners together, HCGC has provided useful and insightful information to the Central Ohio region. This learning has led to effective action in improving healthcare that has included spreading patient centered medical homes and coaching primary care practices, transparently reporting quality measures and highlighting quality improvement opportunities among healthcare providers, creating a regional medical neighborhood and referral infrastructure, supporting patient navigation and patient/family advisory councils, and promoting innovative employer/purchaser models of care. Our staff team are active, engaged experts in healthcare improvement.

Some of our initiatives include Medical Neighborhood, Quality Transparency, Collaborative Learning and Patient-Centered Medical Homes.
